COVID-19 GUEST DECLARATION FORM
As required by the Victorian Government, a COVID-19 Guest Declaration form must be completed and submitted by all persons attending the park.
The form needs to be completed on the day of arrival, not before, and must be submitted prior to check in.
You can include other guests on this form providing they usually reside at the same address as you, and were at the same "Address Last Stayed" as yourself.
The "Address Last Stayed" is the address of the place that you spent last night.
Under "Listed Parties", please list yourself first, then click save. You can add additional guests providing they meet the above criteria.
