As required by the Victorian Government, a COVID-19 Guest Declaration form must be completed and submitted by all persons attending the park.

Important Notes:

  • The form needs to be completed on the day of arrival, not before, and must be submitted prior to check in.

  • You can include other guests on this form providing they usually reside at the same address as you, and have been to all the same locations as as yourself.

  • The "Locations" field is the name and address of all the places that you stayed at during the past 14 days.

  • Under "Listed Parties", please list yourself first, then click save. You can add additional guests providing they meet the above criteria.
