Before riding or entering the Motor Bike Track area, the following indemnity form must be completed by all persons, whether actually riding on the track or just observing.

Important Notes:

• The "This agreement dated the" field is the date that you are completing and submitting the form.

• The "For the period covering the" field is the date that you are first entering the Motor Bike Track area.

• The "And the" field is the date that you will be entering the Motor Bike Track area for the last time.

• You must list, separately, the first name, last name and date of birth of each person entering the Motor Bike Track area, whether riding or just observing.

• You may only list people who are immediate family and normally reside at the same address.
